The minimum premium for kennels and trainers is $500.00. A premium check for the full amount can be made payable to Sportsmen's Insurance Agency Plan, Inc. or, if you choose, quarterly installments can be made. The first installment should be made payable and mailed to Sportsmen's Insurance Agency Plan, Inc., thereafter; invoices will come directly from the Company.
PLEASE NOTE: THIS IS NOT A HOMEOWNERS POLICY. KENNEL AREA MUST BE DETACHED FROM THE RESIDENCE.

One you have the application, we suggest you return the application to us in a timely manner because in most states we will need to obtain clearance from the underwriter. Once we have heard from the underwriter you will be notified.
** Please fill out the application completely, including the amounts of coverage you require. We also need too know the square footage of the buildings and the amount of personal property to be covered and we require a photo of the buildings to be covered.
